This registration is no longer valid and you need to register again as explained above.
This is because of new legislation that came into force in December 2016 regulating organ donation.

Following your death, you can donate your organs to be used for transplantation. If you donate your organs, you  will giving a better life or a new life to somebody in need. Organ donation is not an easy issue to discuss and it is even more difficult for the next-of-kin to give their consent for the use of organs of their beloved ones for transplantation. But if you give your consent by signing up as an organ donor, you will ease pressure on the next-of-kin by giving your decision as a clear indication that you are in favour of transplantation.

You may decide to donate either a particular organ or you may choose to offer any part of your body for transplantation. This can make it possible for more than one person to benefit from your donation.

Once you sign up as an organ donor, your particulars will be included in the Donors Register and will remain in this register unless you change your mind. However, you can change your mind at at any time and can choose to remove yourself as a organ donor if that is what you wish.
